Job Description

General Summary of Position
The PGY-1 pharmacy residency position is designed to produce a well-rounded pharmacy healthcare professional through exposure to multiple acute care experiences in various settings that expand upon fundamental knowledge for general inpatient pharmacy services. The program builds on the Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for post graduate year two (PGY-2) pharmacy residency training.


Primary Duties and Responsibilities

  • PGY-1 Core and Elective Experience Acute Patient Care: Under supervision of Residency Coordinator or designee, completes all requirements for ASHP Residency Program, including but not limited to the following: Evaluates and monitors drug therapy of assigned patient Responds to evaluative and non-evaluative drug information requests Evaluates and dispenses medication order Prepares intravenous admixtures and compounds sterile products Compounds extemporaneous product Dispenses controlled substances and maintains perpetual inventory
  • Performs nursing unit inspection Interviews patients to obtain medication history and counsels patients on the safe and appropriate use of medications Provides coverage for staffing as required for successful completion of educational aspects of Residency Program.
  • Drug Information and Drug Use Policy Development: Under supervision of Residency Coordinator or designee, completes all requirements for ASHP Residency Program, including but not limited to the following: Responds to evaluative and non-evaluative drug information request
  • Performs drug use evaluations. Prepares monographs and other reports (medication errors, adverse drug reactions, etc.) as required Participates in the development, implementation, and management of clinical practice guidelines and therapeutic interchange programs Provides in-service education for pharmacists and other healthcare professionals. Participates in quality improvement and medication use review activities. Includes collecting data, monitoring processes, analyzing data and generating reports. Practice Management:
  • Under supervision of Residency Coordinator or designee, completes all requirements for ASHP Residency Program, including but not limited to the following: Participates in the conduct of quality assurance activities Participates in the development of the department budget and other financial reports Participates in the development of departmental statistical report Completes a research project.
  • Participates in formulary management, including strategic planning for drug shortages. Participates in multidisciplinary quality and service improvement teams. Participates in meetings and on committees and represents the department and hospital in community outreach efforts. Performs other duties as assigned. Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.
  • Clinical: Provides clinical counseling, training and education to assist physicians and healthcare providers for positive outcomes. Plans, directs and coordinates the clinical aspects of pharmaceutical services for patients.
  • Reviews patients' profile to determine possible contraindications with the prescription. Develops clinical pharmacy standards related to and develops organizational policies/protocols to ensure quality of medical therapy. Monitor patients for therapeutic and adverse events.
  • Assures compliance of prescribers with clinical practice guidelines, minimizes non-formulary drug use, eliminates therapeutic duplication and makes recommendations for appropriate therapy Monitors drug therapy of assigned patients. Identifies areas for optimum drug therapy and assures drugs/dosages are accurate based on the therapeutic intent and the patients' physiologic functions.
  • Participates autonomously in patient care rounds. Reviews patient specific data to evaluate patient's status and medication needs and develops a therapeutic plan.
  • Education: Provides education to patients and medical staff on prescribed medications. Provides drug and clinical information to other healthcare professionals in response to specific questions relating to new evolving issues related to the care of patients.
  • Provides product information regarding products, product availability, use, dose and adverse effects to patients, physicians, nurses to assure safe and effective utilization of meds and IVs. Develops and conducts in-services for nurses, physicians, pharmacists and technicians.

  • Development: Maintains current knowledge, certification, and licensure. Identifies learning needs in self and initiates actions to address these education needs. Attends continuing education programs in order to maintain licensure. Attends hospital sponsored training programs as required by the department. Completes all mandatory requirements, certifications, and licensure within allotted timeframe for completion (License, PPD, Competencies etc.)
  • Organizational knowledge: Works effectively within and between departments. Participates on inter-departmental teams and committees, communicating unit knowledge as appropriate. Provides department/unit with information and knowledge acquired during participation with interdepartmental teams and committees.
  • Demonstrates a constructive approach during all interactions with staff, supervisors, and managers both inside and outside the unit. Coordinate patient care with inpatient colleagues for patients. Maintains a close working relationship with nursing and other clinical staff, including physicians in order to coordinate the safe and effective delivery of pharmacy services. Provide education to patients.
  • Department/Unit Specific Functions and Duties Pitches in to cover shortages within department as a pharmacist, which includes all duties of a pharmacist. Recommends improvements to department procedures and intra/inter departmental processes

  • Minimum Qualifications
    Education

    • Pharm D. from an ACPE-accredited school of pharmacy required

    Experience

    • Prior work experience not required

    Licenses and Certifications

    • Maryland Pharmacy license within 120 Days required

    Knowledge, Skills, and Abilities

    • Ability to communicate effectively with all internal and external customers.
    • Moderate Computer skills to perform prescription entries.
    • Experience with computerized Pharmacy information systems
    • HIPAA policies on Confidentiality
