Responsibilities

THERAPIST- BEHAVIORAL HEALTH

Horizon Health is seeking an OTP Counselor/ Therapist for our clinic in Highpoint, NC. The OTP Counselor/ Therapist is responsible for providing counseling, educations, and referral services to enrolled patients and their families as defined by OTP protocols. Counseling services include individual, group, and family counseling.

RESPONSIBILITIES:

  • Conduct Individual, group, and family therapy.
  • Complete social services assessment on patients enrolled in the OTP
  • Develop patient-specific short term goals and social services interventions and documents on treatment plan
  • Complete social services updates to the treatment plan as assigned
  • Conducts patient education programs according to needs identified, including health and wellness education
  • Participates in treatment team meetings, providing feedback to the interdisciplinary team
  • Make and maintain contacts with families, referral sources, and significant others as per policy.
  • Make necessary referrals as outlined by OTP standards

*Required to work flexible hours and weekends*

Qualifications

  • Bachelor's deree required
  • Previous knowledge and experience working with indivudals with Substance Use Disorders, with direct experience in family, individual, and group therapy, case management, crisis intervention and treatment skills.
  • License/ Credentials: must have one of the following: CADAC II, CADAC IV, LMSW with MATS, LMHC, LMHCA with MATS, LCSW, LSW with MATS, LCAC, LAC, LAC-A, MAC, NCAC I, NCAC II, NCAAC
