Overview

Coordinates and manages the implementation, education, data, and certification of Quality Clinical programs of focused populations. Provides leadership and coordination of evidence-based activities to support improved care for patients through early identification and treatment. Track outcomes through data collection for the focused patient population through this program that will improve patient experience, decrease patient harm, decrease cost, and improve physician engagement.

Responsibilities

Patient Care Activity
a. Facilitates communication and coordination of care across the care continuum
b. Quality education of all stake holders
c. Serves as quality consultant for the patient population
d. Provides real time feedback and input toward identifying successes and improvement opportunities to stakeholder
e. Collaborates with physicians and staff in an effort to improve outcomes for patients
Coordinates Quality Improvement initiatives for focused patient population
A. Maintains knowledge of protocols and evidence based care
B. Supports an environment of evidence based practices/research/quality improvement
C. Identifies and coordinates process improvement activities by assisting specific tasks intended to improve performance to various departments, and conducting the appropriate follow-up to ensure successful outcomes
D. Collects, analyzes and interprets data pertaining to focused population patients for quality improvement
E. Disseminates information and research findings though formal and informal presentations within and outside Cone Health System
F. Participates in the development, implementation, and evaluation of protocols, policies, and practice standards

Education
a. Coaches physicians on documentation and compliance of the quality guidelines
b. Provides orientation and in services on quality of care for new staff
c. Participates in the evidence based educational activities for patients, families, staff, students, physicians and the community relative to changes/areas or improvement
d. Coordinates participation in state/national seminars/conferences
e. Develops and collaborates on stakeholder teaching materials, and assists in staff development regarding patient care and the development of related materials.
Qualifications

EDUCATION:
Required:
Graduate of an accredited School of Nursing:
BSN required
Preferred:
Master of Nursing degree in process with defined graduation date will be considered.

EXPERIENCE:
Required:
Experience with leading performance improvement. 5-7 years? experience in acute care setting

LICENSURE/CERTIFICATION/REGISTRY/LISTING:
Required:
Current valid North Carolina Registered Nurse License
