Overview

The Practice Liaison is responsible for the establishment and ongoing management of relationships with Triad HealthCare Network (?THN?) providers and their office staff as well as vendors and payers regarding the implementation and usage of THN resources deployed to advance THN quality and clinical integration initiatives. This includes assisting with the determination of necessary provider recruitment, onboarding and initial education to ensure network adequacy; serving as the primary liaison with THN providers and their staff to coordinate the implementation, training, support and ongoing maintenance of all such THN resources as well as the tracking and reporting of the usage of these resources. This position also is responsible to provide support and assistance to THN and THN providers to achieve targets set forth in established THN incentive programs in addition to achievement of recognition and/or designation from national accrediting bodies (NCQA, Bridges to Excellence, etc.), THN contracted insurance companies or other respective entities for accomplishments of disease management, population health management, medical cost management and adoption of Patient-Centered Medical Home (PCMH) principles. This may include, but is not limited to, coordinating, tracking and reporting adherence to established benchmarks and observing and assisting with process design recommendations to achieve higher compliance and performance. This position will serve THN providers and their staff with contracted payer claims resolution and provide management and support of divisional meetings to report and review quality and cost utilization performance. The practice liaison will also serve as the primary channel of communication between THN and THN providers and their office staff.

This position does require travel - 80% travel between practices and 20% at home office.

Responsibilities

Recruiting, onboarding and orienting new practices, including establishing and maintaining physician and office relationships. Monitors and communicates trends and issues that may affect THN physician relationships.

Analyzes and provides hands-on support for moderate to complex inquiries. Determines appropriate technical area or vendor to resolve a reported issue and coordinates with other respective areas, as needed to achieve resolution. Recommends process changes and works with the THN team to improve the implementation, maintenance and support of hardware, software, and/or services deployed by THN.

Collaborates with THN team to support and assist practices to achieve targets and benchmarks as established by payer incentive programs. Communicates timelines to practices regarding required and optional areas of competency development and educational opportunities.

Deploys, assists and coordinates the implementation, training, support, and ongoing maintenance of all THN resources to practices, including, but not limited to, IT, PCMH and Clinical Integration Initiatives.

Promotes individual professional growth and development by meeting requirements for mandatory/continuing education, skills competency, and supports department-based goals which contribute to the success of the organization.

Provides regular, consistent feedback to THN and THN physicians on measurable results by analyzing practice workflows and processes and recommending changes to improve the implementation, maintenance and support of THN resources and services.

Works with the vendors and appropriate THN staff and departments to develop standardized policies and procedures and deploy best practices in the utilization of deployed THN resources. Demonstrates the ability to lead and influence others positively, especially in the practice setting, to achieve desired goals of THN.

Analyzes and assists with resolution of THN contracted payer claims issues. Provides management, analysis of quality and cost utilization performance for divisions. Coordinates all communication between THN and THN providers and their office staff.

Qualifications

EDUCATION:
High School Diploma or equivalent Required; Bachelors Preferred
An equivalent combination of education and experience may be considered.

EXPERIENCE:
5 years relevant experience.
