Job Description

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Supervise, evaluate, and train Life Skills Coordinators to ensure the health, safety, well-being and Individual Service Plan goals of each day program participants are implemented. Ensure that a ratio of one staff to every 10 participants is met and that staff engage/support day program participants develop and attain life goals that they have identified as meaningful and desirable to them. Oversee staff with curriculum and implementation of technology.
  • Identify, assess, and expand community partnerships opportunities for the day program participants and recruit volunteers to support day programing activities. Meet/Assist Family and Friend's Auxiliary group to plan holiday events/fundraisers.
  • Stabilize enrollment to 22-24 participants by recruiting NC Innovation Waiver participant's receiving individual and/or day support group services. Marketing the day program to Alliance Care Coordinators and market private pay options.
  • Assist with implementation of social enterprise opportunities for day program participants to sell produce at Farmer's Market as well as gift cards.
  • Inter face with ServiceSource Rehab Program Director, Care Coordinators, the participant, and their family members/guardians to develop their Annual Plan, write/review their quarterly progress notes, complete daily grids, task analyses, and weekly schedule of activities for the participant's and family.
  • Performs related duties and responsibilities as assigned/requested.

ADDITIONAL DUTIES AND RESPONSIBILITIES

  • Maintain case files and enter data which includes development of participant's Individual Service Plan (ISP), quarterly progress notes, weekly schedule of activities, reporting in Intelex incidents/accidents, and daily attendance sheets
  • Maintain positive relationships with families, volunteers, care providers, community partners, stakeholders, Alliance Health, and other ServiceSource Regional Offices
  • Lead weekly staff meetings with day program staff
  • Attend required ServiceSource meetings/training and other trainings/conferences in the community
  • Monitor and assist with the boarding and exiting of participants from their mode of transportation
  • Implement and monitor goals and role model positive behavioral interventions with participants/staff
  • Oversee/recruit volunteers and work closely with ServiceSource director of Community Relations and Philanthropy
  • Understand and utilize Alliance Alpha authorization/billing system in order reauthorize day program services from Alliance Health
  • Provide support and documentation during Alliance Health and CARF audits
  • Participate in yearly company strategic planning (SWOT Analysis

QUALIFICATIONS: EDUCATION, EXPERIENCE AND CERTIFICATION(S)

  • BA in related human services filed, 3 years related experience working with adults with disabilities, including supervisory experience or equivalent combination of education and experience.
  • Must possess a valid driver's license, good driving record and be willing to transport participants in company vans/car.
