Elwyn Adult Behavioral Health Services (formerly Fellowship Health Resources) fosters hope and recovery. Elwyn provides behavioral health services to improve the quality of life for individuals living with mental illness and addictions, serving over 7,000 individuals across 6 states. The cornerstone of our success is Elwyn's deep and longstanding commitment to the principles of recovery and the inclusion of peers in the recovery process.


Elwyn Adult Behavioral Health Services is seeking a compassionate and dedicated Case Manager to join our innovative Community Support Team (CST) program based in Raleigh, NC. This is an exciting opportunity to be part of a groundbreaking initiative that empowers individuals with severe and persistent mental illness to thrive within their communities.

The CST program is a new chapter for Elwyn, and you'll play a vital role in its success. This program offers individualized support to 48 individuals in Wake County, focusing on recovery and resiliency. We believe in the power of person-centered care and natural support to help individuals manage their behavioral health needs, improve daily living activities, and achieve greater independence. Ultimately, the CST program aims to equip clients with the tools and support they need to transition to lower levels of care and live fulfilling lives within their communities.

$40,000 - $50,000 / year, comprehensive benefits
Monday - Friday, 8 am - 4 pm, Raleigh, NC, and community outreach

Empower Recovery, Build Independence

  • Develop trusting relationships with clients, working collaboratively to establish person-centered goals and individualized support plans.
  • Empower clients by providing guidance, support, and resources to manage their mental health needs, build independence, and improve daily living activities.
  • Connect clients with essential resources within the community, fostering a strong support network that promotes well-being and recovery.
  • Utilize evidence-based practices to guide interventions and empower clients to develop self-management skills, fostering long-term resilience.
  • Collaborate with a dedicated team of professionals, including mental health clinicians and community partners, to ensure comprehensive and coordinated care.


Qualifications

  • Bachelor's degree in a Human Services field, such as Social Work, Psychology, Rehabilitative Counseling, or a closely related discipline (Public Health, Nursing).
  • At least two years of experience in human services, preferably in case management.
  • Strong understanding of recovery-oriented and person-centered care principles.
  • The ability to establish and maintain effective therapeutic relationships and creatively engage clients in discussions and planning around recovery and personalized goals.
  • A thorough understanding of the clinical symptoms and behavioral aspects of SPMI.
  • Experience with crisis intervention principles and practices to effectively de-escalate situations.
  • Knowledge of prescribed medications used in the program, their effects, and the ability to monitor client self-medication.
  • Ability to manage a caseload effectively and prioritize tasks in a fast-paced environment.
  • Proficient in documentation and record-keeping, with experience in electronic health record (EHR) systems a plus.
  • A valid driver's license in good standing.
